Again, This is for patients who already have an appointment. You will not be able to schedule an appointment through the portal
Important: Your data entries cannot be stored or saved and will be lost if you try to register before you already have an appointment date/time and know who your provider will be.
If you are an adult making an appointment for a child, please also download and complete the permission for child treatment form, which must be signed by both parents, as well as the intake and consent-for-treatment forms.
Please bring forms to your first appointment.
